When I chose to pursue plastic surgery, it was not just a career decision — it was the continuation of a story that began before me. My father, Dr. Walter Zamarian, was one of the pioneers of the speciality in northern Parana. I grew up seeing how surgical care can affect appearance, confidence, and function when it is responsibly indicated. This experience shaped my vision of plastic surgery as a field that requires technical judgement, proportion, and respect for each patient's identity. As a plastic surgeon in Londrina, Brazil, I combine this family legacy with specialised training in Brazil and abroad.

Today, after more than 8,000 procedures and two decades of practice, I keep this same purpose alive. From Cornélio Procópio to Rio de Janeiro: the journey of training

I was born in Cornélio Procópio, in the interior of Paraná. At just two years old, I moved to Rio de Janeiro — my father was completing his specialisation with the legendary Professor Ivo Pitanguy. Although I was too young to understand, that period planted a seed. Years later, when it was my turn to choose a speciality, the path seemed natural.

I graduated in Medicine from UEL and completed my residency in General Surgery at the same institution. A decisive stage in my training came when I was accepted at the Ivo Pitanguy Clinic and in the 38th Ward of the Santa Casa de Misericórdia do Rio de Janeiro, through PUC-RIO. It was three years of immersion in one of the most influential schools in Brazilian plastic surgery.

Professor Pitanguy did not just teach technique. He conveyed a philosophy: each face tells a story, and the surgeon must know how to read it before modifying it. I carry this teaching into every consultation.

The pursuit of improvement never ends

The training in Rio was solid, and I continued to update my practice through further study and surgical observation. Over the years, I repeatedly travelled to the United States to train with surgeons recognised for specific technical contributions to the speciality:

  • I learned the nuances of deep facial rejuvenation with specialists who abandoned the superficial techniques of the 90s
  • I mastered female intimate surgery directly with Dr. Gary Alter, in Los Angeles — the doctor who created the Wedge technique, now considered the gold standard worldwide for labiaplasty
  • I deepened my knowledge in functional rhinoplasty, integrating aesthetics and breathing into a single approach

This combination — Brazilian technical rigour with selected international training — supports a more complete discussion of indications, limits, risks, and alternatives for each procedure.

What sets me apart: the regenerative approach

Over time, I realised that durable facial rejuvenation is not based on simply "stretching" the skin, but on restoring support, volume, and proportion when this is appropriate for the patient. This perception changed my practice.

The science behind fat grafting

When I transplant fat from the patient's own body to the face, the goal is not simply to fill grooves. Human fat contains mesenchymal stem cells and stromal vascular fraction components that are studied for regenerative potential. In appropriately selected cases, fat grafting can support volume restoration and tissue quality.

Patients may report improvement in texture, firmness, or radiance after recovery, although the degree of change varies. This is why I consider fat grafting an important component of selected facial rejuvenation plans.

Face and neck as a unit

Another principle I advocate: it makes no sense to rejuvenate the face while ignoring the neck. Ageing does not respect anatomical boundaries. Therefore, in recommended cases, I combine facial work with neck correction in a single surgical time. The result is a natural transition, without that "break" between a youthful face and an aged neck.

Rhinoplasty: when less trauma means better results

Rhinoplasty has always been one of the most challenging procedures in plastic surgery. Many patients come to the clinic with apprehension — they have heard stories of prolonged swelling, intense bruising, weeks of discomfort.

I use a different approach in selected cases: controlled micro-perforations instead of traditional bone fractures. The bone is remodelled with millimetric precision, aiming to reduce unnecessary trauma and swelling while preserving structural control.

Reducing trauma does not mean simplifying the operation. My rhinoplasty planning is comprehensive: I assess the dorsum, the tip, the base, and — when necessary — the septum and turbinates. The goal is a nose that is balanced with the face and supports breathing whenever functional correction is indicated.

Intimate surgery: a topic that deserves seriousness

Female intimate surgery is still surrounded by taboos. Some women experience physical discomfort, asymmetry, or aesthetic concerns, and selected cases may benefit from a careful surgical assessment.

I was one of the first Brazilian surgeons to train directly with Dr. Gary Alter, internationally recognised for female intimate surgery. My practice includes labiaplasty (reduction of the labia minora), labia majora reduction, asymmetry correction, and clitoral procedures. Each case requires a personalised approach — there is no "one technique" that fits all.

I treat this topic with the same seriousness and technical rigour that I dedicate to the face, including privacy, anatomy, function, expectations, and risks.

Beyond borders: fluency in five languages

Since my teenage years, I have cultivated a fascination for foreign languages. I studied English, Spanish, French, and Italian — initially out of curiosity, then out of professional necessity. Today, this skill allows me to see patients from various nationalities without intermediaries.

I regularly receive patients from the United States, Europe, Japan, and Latin American countries. Direct communication, without translators, makes a difference: I can understand nuances, cultural expectations, specific fears. And the patient feels more secure knowing they will be understood.

For those coming from afar, my team organises logistics such as accommodation near the clinic, transport, and post-operative follow-up planning. Distance requires careful preparation, especially for surgical recovery and return travel.

Contribution to medicine: the Burn Centre of Londrina

Not everything in my journey has been aesthetic surgery. I was one of the founders of the Burn Centre of the University Hospital of Londrina, applying the experience I gained during my internship at the Andaraí Hospital in Rio de Janeiro.

Treating severe burns is a very different field from aesthetic plastic surgery, but it taught me valuable lessons: the importance of meticulous planning, the need to anticipate complications, and the value of a well-trained team. These lessons shaped my approach to risk management — even in procedures considered less complex.

Hospital structure and risk management

I operate at the Hospital do Coração – Unidade Bela Suíça, in Brazil. This is a deliberate choice: I prefer a hospital environment with appropriate support structure for elective plastic surgery.

What this choice represents in practice:

  • ICU available 24 hours — for any emergency, even in procedures considered simple
  • Anaesthetists with specific experience in plastic surgery — professionals who have worked with me for years and know the particularities of each procedure
  • Total intravenous anaesthesia (TIVA) — an advanced technique that provides a smoother awakening, less nausea, and faster recovery
  • Private rooms — comfortable accommodations for immediate postoperative care, with continuous monitoring

Safety is an obligation in surgical care, not a marketing claim. The structure where surgery is performed matters for risk management, monitoring, and the patient's peace of mind.
