Mons pubis, pubic mound or "VW bonnet"

The little fat below the abdomen and above the intimate area is called the mons pubis, or pubic mound. Commonly known as "VW bonnet", excess fat in this area can be bothersome in various situations: in a bikini, in gym trousers or in any tight clothing. Factors such as genetics, ageing, hormonal fluctuations, and significant weight loss — including post-bariatric — can contribute to this fat accumulation.

I treat this excess fat with a simple and effective liposuction. I use the tumescent technique with fine cannulas, specially adapted to this delicate area, which allows for precise aspiration with minimal bleeding. The procedure is quick, smooth, and has a short recovery — my patients are discharged on the same day and can return to work the next day.

Indication

I recommend pubic liposuction for patients with excess localized fat in the mons pubis. Both men and women can benefit from the procedure. Pre-operative

The consultation

In the consultation, I assess the pubic region to identify excess fat on the mons pubis. If there is associated skin laxity, I may recommend a mons pubis lift (pubopexy). If the patient wishes to evaluate the intimate areas more broadly, mons pubis liposuction can be performed in conjunction with other surgeries, such as labiaplasty, reduction of the labia majora, or even abdominoplasty — a combination I frequently perform to harmonise the entire lower abdominal region.

The anaesthesia

For pubic liposuction, I use total intravenous general anaesthesia, the same that I employ in all my procedures. This way, the patient undergoes a safe, painless procedure with rapid anaesthetic recovery, being discharged on the same day.

The surgery

I perform mons pubis liposuction through two small openings of about 1 cm, through which I introduce a tumescent solution containing saline and adrenaline, to minimise bleeding. Then, I use a thin liposuction cannula and remove the fat from its two layers: the superficial and the deep. It is essential to aspirate both layers to achieve the best possible result. The two openings are closed with an absorbable internal stitch and cyanoacrylate glue, eliminating the need for stitch removal afterwards. The scars become practically imperceptible after complete healing.

Post-operative

The recovery from pubic liposuction is quite smooth. Being discharged on the same day as the surgery, I advise the patient to wear a compression garment for two months and to start lymphatic drainage on the fifth day and endermology on the tenth day. I recommend a month without physical activity and without sexual relations for the same period. In the first three days, it is important to drink plenty of fluids to maintain adequate hydration. I also prescribe antibiotics, anti-inflammatories, and analgesics for the post-operative period.

Final result

Like all liposuctions, the results begin to become quite apparent around the third week, but it is at two months that the area significantly reduces in swelling, coinciding with the first post-operative photos. The final result can be appreciated around the sixth month. The results are long-lasting, provided the patient maintains a stable weight — the removed fat cells do not return.
