Mons pubis, pubic mound or "VW bonnet"

Pre-operative
The consultation
In the consultation, I assess the pubic region to identify excess fat on the mons pubis. If there is accompanying skin laxity, I may recommend a mons pubis lift (pubopexy). If the patient wishes to evaluate the intimate areas more broadly, pubic liposuction can be performed in conjunction with other female intimate surgery procedures, such as labiaplasty, labia majora reduction, or even abdominoplasty -- a combination I frequently perform to balance the entire lower abdominal region.
Examinations
For pubic liposuction, I request some examinations, such as:
- Complete blood count;
- PT with INR + APTT;
- Urea;
- Creatinine;
- Fasting blood glucose;
- Total proteins and fractions
- Vitamin D
- Vitamin C
- Urinalysis;
- ECG;
- Pre-operative cardiac assessment.
The anaesthesia
For pubic liposuction, I use total intravenous general anaesthesia, the same that I employ in all my procedures. This way, the patient undergoes a safe, painless procedure with rapid anaesthetic recovery, being discharged on the same day.
The surgery
I perform pubic liposuction through two small openings of about 1 cm, through which I introduce a tumescent solution containing saline and adrenaline, to minimise bleeding. Then, I use a thin liposuction cannula and remove the fat from its two layers: the superficial and the deep. It is essential to aspirate both layers to achieve the best possible mons pubis contouring result. The two openings are closed with an absorbable internal suture and cyanoacrylate glue, eliminating the need for suture removal afterwards. The scars become practically imperceptible after complete healing.
Post-operative
The recovery from pubic liposuction is quite smooth. Being discharged on the same day as the surgery, I advise the patient to wear a compression garment for two months and to start lymphatic drainage on the fifth day and endermology on the tenth day. I recommend a month without physical activity and without sexual activity for the same period. In the first three days, it is important to drink plenty of fluids to maintain adequate hydration. I also prescribe antibiotics, anti-inflammatories, and analgesics for the post-operative period.
Final result
Like all liposuctions, the results begin to become quite apparent around the third week, but it is at two months that the area significantly reduces in swelling, coinciding with the first post-operative photos. The final result can be appreciated around the sixth month. The results are long-lasting, provided the patient maintains a stable weight — the removed fat cells do not return.
