Chin augmentation in Londrina, Brazil: at a glance

What is chin augmentation?

Chin augmentation, or a chin implant, increases the projection of a small chin with a solid silicone implant placed through an incision inside the mouth. It is indicated when the bite is balanced and the deficiency lies in the chin itself. It does not correct the bite, it is not a substitute for orthognathic (corrective jaw) surgery, and it does not treat a sagging neck.

  • Technique: a solid silicone implant placed through the mouth, with no planned external scar; I don’t perform bony genioplasty or chin reduction.
  • Recovery: total intravenous anesthesia (TIVA); surgery takes about 25 minutes; a liquid diet for about five days; light activity after one month, any activity after two.
  • Risks: infection, implant shifting, changes in sensation, bone resorption, and the need for revision; complications in 15.7% of implants in a review of 54 studies.
  • Combined procedures: it can be done along with a facelift when both needs are present.
  • Results: no set date for replacing the implant and no guarantee; follow-up visits for one year.

What bothers you about your profile may not be where you think

When you show me a photo taken from the side, I look at more than the spot you point to. A chin that lacks projection can change how the nose, the lower lip, and the transition to the neck come across. In other people, the projection is fine and the concern comes mainly from sagging.

The consultation is where we sort out these possibilities. I want to understand whether you want a stronger profile, whether you notice an asymmetry, or whether you expect surgery to improve the area under your jawline. Each of these goals may call for a different answer.

My aim is not to give everyone a stronger chin. The projection has to work with the rest of your face, including from the front and when you move.

Three situations that shouldn’t be confused

A small chin with a balanced bite

Chin augmentation can be considered when the lack of projection is concentrated in the chin.

A jaw or bite problem

You may need an evaluation by an oral and maxillofacial surgeon and an orthodontist. An implant does not reposition the whole lower jaw or correct your bite.

Neck skin and tissues

Treatment may involve a neck procedure or a facelift. Adding projection to the chin does not remove sagging.

I examine how your teeth fit together and ask about any dental treatment you have had. If there is any doubt about the underlying structure, I refer you to the right specialist before we choose an implant. This avoids a surface-level fix for a problem that needs a different plan.

You don’t need to tell these situations apart on your own. Bring the photo that bothers you and explain what you would like to look different; the diagnosis is part of my job.

How I plan chin augmentation with an implant

I use a solid silicone implant, which is different from an injectable gel. The incision is on the inside of the lower lip. I create a pocket to hold the implant over the chin bone and adapt its shape to the anatomy I see. With the technique I use, there are no screws; stability depends on how that pocket is prepared and on healing.

Choosing the size isn’t just a matter of gender or a size number. I look at width, projection, and the transition to the jawline, along with the preferences you have shared with me. A change in profile still has to make sense when we look at your face from the front.

The incision leaves a scar inside the mouth. Avoiding a planned external scar is a feature of this approach, but it does not eliminate the risk of infection, implant shifting, or healing problems. I explain these differences before you decide.

The surgery is done in a hospital, under total intravenous anesthesia (TIVA), the same anesthesia I use for almost all my surgeries. In my hands, chin augmentation takes about 25 minutes. When it is done without a facelift, you can go home the same day if you wish; since I operate on these cases in the afternoon, discharge is around 10 p.m. When it is combined with a facelift, your hospital stay is the same as for the facelift.

Shall we look at your profile as a whole?

At the consultation, I will clarify how much of what bothers you comes from the chin and what chin augmentation can add.

The chin’s role in a facial rejuvenation plan

When I plan a deep plane facelift, the underlying bone structure matters. Repositioning tissue can improve contour, but it does not create the same forward projection an implant provides. On the other hand, enlarging the chin does not reposition the face the way a facelift does.

When both needs are present, we can discuss combining the two. A deep neck lift also calls for its own evaluation: fat, skin, and the structures of the neck shouldn’t be treated as if the problem were simply a small chin.

I show you the role of each procedure and what it leaves unresolved. The advantage of a plan you understand is that you know why each procedure is there, instead of accepting a list of procedure names that has nothing to do with your concern.

Implant or bone surgery: what goes into the decision

Bony genioplasty repositions a segment of the chin bone, whereas an implant adds projection on top of the existing structure. They are different approaches, each with its own indications and risks. When a case calls for bone surgery, I refer the patient to a specialist for evaluation; I don’t perform this procedure.

A review by Liao and colleagues included 39 publications on chin implants and found that complications varied mainly with the surgical approach; among materials, the difference was in altered sensation. Most of the studies were retrospective, which limits any conclusion that one option is superior. Review of implant materials and techniques.

In 2025, a review compared implants with bony genioplasty across seven studies totaling 1,126 patients. Both approaches showed favorable outcomes, with high satisfaction in both groups (trending higher with genioplasty), different risk profiles, and limits on how directly they could be compared. These data support choosing based on the indication, not the idea that one solution fits every profile. Comparison of implants and genioplasty.

What the numbers in the literature show

A systematic review by Oranges and colleagues included 54 studies and 4,897 patients. Complications occurred in 15.7% of implants and 19.7% of osteotomies. Transient changes involving the mental nerve, which supplies sensation to the lower lip and chin, occurred in 2.4% of implants and 16.4% of osteotomies. Review of chin augmentation techniques.

In the review by Liao and colleagues, the rate of altered sensation with silicone was 0.4%. The intraoral approach had more implant removals than the external approach, 1.5% versus 0.5%, but less asymmetry, 0.7% versus 7.5%. In a retrospective study of 80 patients, Tabrizi and colleagues found infection to be more common with implants than with bony genioplasty, and less wound separation when the implant was placed from outside the mouth. Study comparing implants and genioplasty.

Each approach trades one risk for another. I choose the intraoral approach to avoid an external scar, and I tell you what that choice involves. Numbers from published series do not predict your individual risk.

Recovery starts with caring for your mouth

Because the incision is inside the mouth, eating and oral hygiene need extra attention. I routinely recommend a liquid diet at first, for about five days, and adjust how you progress based on your healing. The external dressing usually stays on for five to seven days. You’ll receive specific instructions before discharge.

According to the ASPS, after chin augmentation mild swelling may occur for a few days, and patients are usually able to return to work within a week. Your own timeline depends on your work, how you heal, and any combined procedures.

Swelling and temporary changes in sensation can make the contour harder to judge. Don’t press on the implant to check its position, and don’t stop your aftercare because the outside looks healed. Follow-up visits also let me examine the incision inside your mouth.

I usually recommend lying on your back for the first month. Light exercise is allowed after one month, and any physical activity, including contact sports, after two months. When it is combined with a facelift, we follow the overall recovery plan. Follow-up photos help us see how the chin settles, a process that continues beyond the first few days.

If you are traveling to Londrina, arrange your stay and follow-up visits with the team. The patient journey page walks you through each step, so your trip isn’t planned around the surgery date alone.

What to watch for even after the swelling goes down

Risks include infection, bleeding, altered sensation in the chin or lower lip, asymmetry, implant shifting, implant exposure, and the need for removal or revision. The bone in contact with the implant can also resorb. No material eliminates these possibilities.

There is no set date by which every implant must be replaced, but that doesn’t mean you can skip follow-up. A late change in contour, pain, discharge inside the mouth, or exposed implant material should be evaluated. Fever or swelling that increases quickly also means you should contact the team.

If you have already had surgery and are unhappy with the result, bring whatever records you have. A revision evaluation looks at the implant’s position, the scars, and the condition of the bone; adding more volume without understanding the cause can make the problem worse.

Follow-up visits continue for one year, even when recovery seems uneventful.

A 2026 publication described patients with complications from silicone implants who needed different revision strategies. This selected sample does not estimate the risk for all implants, but it shows why removing the implant may be only one part of treatment. When the correction calls for a procedure I don’t perform, I refer the patient to a specialist for evaluation. Study on chin implant revisions.

Your questions deserve a careful answer.

Will chin augmentation fix my bite?

No. A chin implant changes projection locally. Bite problems may require orthodontic treatment and an evaluation for orthognathic surgery.

Do you perform chin reduction?

I don’t perform chin reduction or bony genioplasty. When those approaches seem necessary, I recommend an evaluation with the appropriate specialist.

Will I have a scar under my chin?

With the approach I use, the incision is inside the mouth. There is an internal scar that needs its own care, even without a planned external incision.

Will I see the result right away?

The surgery changes the projection right away, but the dressing and swelling affect how you look at first. The assessment becomes more reliable as the tissues settle.

Can fillers replace an implant?

In some cases, injectables can change the contour, but they differ in how long they last, their risks, and their limitations. I don’t perform filler treatments, and I don’t use this comparison to recommend surgery to someone who doesn’t need it.

How much does it cost?

The quote is individual and depends on the implant, the hospital, anesthesia, and any combined procedures. When chin augmentation is done with a facelift, the chin implant appears as a separate item in the quote. I only see private-pay patients, and the team goes over the itemized quote after your evaluation, once the plan is defined.

If I have the implant removed, will everything go back to how it was?

I can’t guarantee that. Removing or replacing the implant requires another evaluation and another surgery. I examine the tissues, the scars, the implant’s position, and the condition of the bone to explain what a revision may involve. Being able to remove the implant doesn’t make the first operation a trial run without consequences.

Let’s look at your profile, your bite, and how your chin relates to your neck to work out which evaluation is right for you.
