Rhinoplasty: Nose Plastic Surgery

The nose occupies a central position on the face and directly influences the harmony of the face. Small changes to the nose can completely transform the facial aesthetics, as well as improve breathing when there are associated functional problems.

I perform structured open rhinoplasty, a technique that allows for greater precision and control of the result. Unlike old techniques that merely removed cartilage, structured rhinoplasty remodels and strengthens the nasal structures, ensuring more durable and natural results.

The surgery can correct:

Facelift Deep Plane: Deep Rejuvenation

I specialise in the deep plane facelift technique, which repositions the deep tissues of the face (SMAS layer) instead of merely stretching the skin. The complete procedure may include treatment of multiple areas:

Jawline and "bulldog": The drooping of tissues over the jawline (lateral double chin or "bulldog") is treated with precise liposuction and SMAS plication. The result is a defined and youthful jawline.

Neck (Deep Neck Lift): Cervical ageing includes wrinkles, platysma bands (cords in the neck), and accumulation of submental fat. I treat the area with liposuction, platysma plication, and removal of excess skin, restoring the defined cervicomental angle. Recovery: The surgery lasts about 3 to 4 hours. Most patients return to work in 10 to 14 days, already looking rested and natural. The result continues to improve in the first months as the swelling decreases.

Fat Grafting: Rejuvenation with Stem Cells

Fat grafting (facial lipotransfer) is one of the most versatile techniques in modern plastic surgery. In addition to restoring the volume lost with ageing, the transplanted fat contains mesenchymal stem cells that promote real skin rejuvenation.

Blepharoplasty can treat:

Techniques used:

Recovery: The stitches are removed in just 3 days. The swelling and bruising decrease significantly in the first week. Most patients return to social activities in 7 to 10 days.

Mentoplasty: Chin Harmonisation

The chin is fundamental for the balance of the facial profile. Patients with a small, retruded, or poorly projected chin often present disharmony between the nose, lips, and chin, as well as a poorly defined jawline.

The chin augmentation mentoplasty uses a solid, biocompatible, and highly durable silicone implant. The surgery is performed through an intraoral incision (inside the mouth), with no visible scar. The implant is positioned directly on the bone, projecting the soft tissues naturally.

Indications:

  • Small or retruded chin (retrognathism)
  • Facial profile disharmony
  • Complement to rhinoplasty for profile balance
  • Double chin due to lack of bony projection

Recovery: The result is already visible in 5 days. The diet is soft in the first week for comfort. The residual swelling decreases progressively in the first weeks.

Otoplasty: Correction of Protruding Ears

Protruding ears can cause significant embarrassment, especially in children. Otoplasty is a safe surgery that can be performed from the age of 6, when the ear cartilage has developed sufficiently.

What does otoplasty correct:

  • Protruding ears
  • Absence of the anti-helix fold
  • Excessive auricular concha
  • Asymmetries between the ears

The surgery is performed through an incision behind the ear (hidden scar) and consists of reshaping the cartilage to create the natural fold of the anti-helix and, when necessary, reducing the concha. The procedure lasts less than an hour per ear.

Recovery: A protective bandage is used for 30 days (full time in the first week, then only for sleeping). The patient can return to normal activities in a week, avoiding contact sports for 2 months.

Dermabrasion: Treatment of Wrinkles and Scars

Dermabrasion is a deep peeling that removes the superficial layers of the skin in a controlled manner, stimulating the regeneration of new, smoother, and more even skin.

Main indications:

  • Barcode: Vertical wrinkles around the lips
  • Perioral wrinkles: Lip wrinkling due to smoking or ageing
  • Acne scars: Irregularities on the skin surface
  • Traumatic scars: Sequelae from accidents or surgeries

The procedure can be performed alone (outpatient, with local anaesthesia and sedation) or in conjunction with a facelift for complete treatment of perioral ageing.

Recovery: The treated skin forms crusts that peel off in 7 to 10 days. Redness decreases progressively over a few weeks. Strict sun protection is essential for 3 months.

Buccal Fat Removal: Reduction of the Buccal Fat Pad

The Buccal Fat Pad is a structure of fat located in the cheeks, below the cheekbones. When voluminous, it can give the appearance of a "round face" or "baby cheeks" that persists even with weight loss.

Buccal fat removal consists of the partial removal of this fat through a small intraoral incision, resulting in:

  • More sculpted and defined face
  • Enhanced cheekbones
  • More angular facial contour
  • More mature and sophisticated appearance

Important: Buccal fat removal is not indicated for everyone. I carefully assess the facial anatomy of each patient, as in some individuals, removal may cause an early aged appearance. Correct indication is fundamental for a good result.

Recovery: Outpatient procedure with local anaesthesia. Moderate swelling for 5 to 7 days. Soft diet in the first week. Definitive result in 2 to 3 months.

This reduces the number of anaesthetics, optimises recovery, and provides a more complete and integrated result. During the consultation, I evaluate which combinations are safe and indicated for each case.

What is the ideal age to undergo facial plastic surgery?

There is no fixed age. In my practice, I see patients from the age of 20 for rhinoplasty and otoplasty, and from 40-50 years for facelift. The important thing is that there is a real indication and realistic expectations. Each decade of life presents different characteristics that influence the choice of technique.

What is the recovery like after facial plastic surgery?

Recovery varies depending on the procedure performed. Generally, the first 7 to 14 days require more care, with swelling and bruising that decrease progressively. I recommend relative rest in the first week, keeping the head elevated while sleeping, and applying cold compresses.

Do the results of facial surgery look natural?

This is my absolute priority. I use modern techniques such as the Deep Plane lift and structured rhinoplasty precisely because they provide natural results, without that "pulled" or "operated" appearance. My philosophy is to enhance the beauty that already exists, respecting the individuality of each face.

Like any surgery, facial procedures carry risks, although they are rare when performed by a qualified surgeon in an appropriate environment. The most common risks include hematoma, infection, and temporary sensitivity changes. I minimise these risks with careful pre-operative assessment, meticulous surgical technique, and close post-operative follow-up. In my experience with over 8,000 surgeries, the complication rate is very low.

How long do the results of facial surgery last?

The durability depends on the procedure. The Deep Plane lift offers results that last from 10 to 15 years, rhinoplasty and mentoplasty are permanent, and blepharoplasty usually lasts from 7 to 10 years. Facial fat grafting has approximately 60-70% retention of the grafted volume.

Do I need to be hospitalised for facial plastic surgery?

However, facelift and rhinoplasty generally require one night of hospitalisation for greater comfort and safety. When I combine multiple procedures, one night of hospitalisation is standard.
