Pubic Liposuction: Eliminate Stubborn Fat in the Mons Pubis

The soft tissue between the lower abdomen and the genital area is called the mons pubis, often referred to as the "pubic mound" or colloquially as a "FUPA" (Fat Upper Pubic Area). Excess fat in this region can be a source of self-consciousness in swimwear, yoga pants, or any form-fitting clothing. Factors such as genetics, aging, hormonal fluctuations, and significant weight loss -- including after bariatric surgery -- can contribute to this stubborn fat accumulation.

I treat this excess fat with a straightforward and highly effective liposuction procedure. Using the tumescent technique with fine cannulas specially designed for this delicate area, I achieve precise fat removal with minimal bleeding. The procedure is quick and recovery is short -- my patients go home the same day and most can return to work the very next day.

Indication

I recommend pubic liposuction for patients with excess localized fat in the mons pubis. Both men and women can benefit from this procedure. In men, accumulated pubic fat can partially conceal the penis, affecting both appearance and sexual function. In women, it creates a noticeable bulge under clothing and causes aesthetic discomfort. I typically treat patients starting at fifteen to sixteen years of age, once physical development is nearly complete.

I explain pubic liposuction (mons pubis reduction)

Pre-operative

The consultation

During the consultation, I evaluate the pubic area to identify excess localized fat in the mons pubis. If there is associated skin laxity, I may recommend a mons pubis lift (pubic lift). If the patient wishes to address the intimate areas more comprehensively, pubic liposuction can be performed in conjunction with other procedures, such as labiaplasty, labia majora reduction, or even abdominoplasty -- a combination I frequently perform to harmonize the entire lower abdominal region.

Exams

For pubic liposuction, I request some exams, such as:

Anesthesia

For pubic liposuction, I use total intravenous general anesthesia, the same that I employ in all my procedures. This way, the patient undergoes a safe, painless procedure with rapid anesthetic recovery, being discharged the same day.

The surgery

I perform pubic liposuction through two small incisions of approximately 1 cm, through which I introduce a tumescent solution containing saline and epinephrine to minimize bleeding. Then, using a fine liposuction cannula, I remove fat from both layers: the superficial and the deep. It is essential to address both layers to achieve the best possible result. The two incisions are closed with absorbable internal sutures and cyanoacrylate tissue adhesive, eliminating the need for suture removal. The scars become virtually imperceptible after complete healing.

Post-operative

Recovery from pubic liposuction is remarkably smooth. Patients are discharged the same day, and I advise wearing a compression garment for two months. I recommend beginning lymphatic drainage massage on the fifth day and endermologie on the tenth day. I advise one month without physical activity and sexual intercourse. During the first three days, it is important to drink plenty of fluids to maintain proper hydration. I also prescribe antibiotics, anti-inflammatory medication, and pain relievers for the postoperative period.

Final result

As with all liposuction procedures, results begin to become quite apparent around the third week. At two months, the area has significantly less swelling, and this is when I take the first postoperative photographs. The final result can be fully appreciated around the sixth month. Results are long-lasting, provided the patient maintains a stable weight -- the removed fat cells do not regenerate.

Frequently asked questions about pubic liposuction

What is a FUPA?

FUPA stands for "Fat Upper Pubic Area" and refers to excess fat in the mons pubis, the area just above the genitalia. In my practice, I see many patients who are bothered by this prominent bulge, visible under tight clothing, swimwear, and workout leggings.

How do you perform pubic liposuction?

I perform the procedure through two small incisions of approximately 1 cm. First, I infiltrate a tumescent solution with saline and epinephrine to minimize bleeding. Then, I use fine cannulas to aspirate fat from both the superficial and deep layers, ensuring the best possible result.

Is pubic liposuction permanent?

Yes, in my experience, the results are long-lasting. The fat cells that I remove do not return. However, I advise my patients to maintain a stable weight, as the remaining cells can increase in volume with significant weight gain.

Is the scar from pubic liposuction visible?

In the vast majority of cases, no. I close the incisions with absorbable internal sutures and cyanoacrylate tissue adhesive, eliminating the need for suture removal. The scars become virtually imperceptible after complete healing.

Can pubic liposuction be combined with other surgeries?

Yes, and this is a combination I frequently perform. Depending on the case, I combine pubic liposuction with a mons pubis lift, labiaplasty, labia majora reduction, or abdominoplasty -- especially to create a harmonious contour throughout the entire lower abdominal region.

Can men also have pubic liposuction?

Yes. In men, accumulated pubic fat can partially conceal the penis, impairing both appearance and sexual function. In my experience, male pubic liposuction yields very satisfying results, significantly improving the contour of the area.

What type of anesthesia is used?

I use total intravenous general anesthesia (TIVA), the same approach I employ in all my procedures. This ensures a safe, painless surgery with rapid anesthetic recovery, and patients are discharged the same day.

What is the recovery like after pubic liposuction?

Recovery is remarkably smooth. I recommend that my patients wear a compression garment for two months, begin lymphatic drainage massage on the fifth day, and endermologie on the tenth day. I advise one month without physical activity and sexual intercourse. Most patients return to work the very next day.

When do the final results appear?

In my experience, results begin to become apparent around the third week. At two months, the area has significantly less swelling -- this is when I take the first postoperative photographs. The final result can be fully appreciated around the sixth month.

What is the minimum age for pubic liposuction?

I typically treat patients starting at age fifteen to sixteen, once physical development is nearly complete. Each case is evaluated individually during the consultation so that I can recommend the best timing and approach.
