## Brow lift in Londrina, Brazil: at a glance

### What is a brow lift?

A brow lift repositions brows that have dropped and weigh down the eyes. It doesn’t treat excess eyelid skin or eyelid ptosis, each of which has its own diagnosis. I use an open technique, with an incision along the brow, always combined with upper blepharoplasty.

- Who it’s for: a drooping outer brow that adds to the heaviness; I don’t offer the procedure on its own.

- Technique: mini-Castañares, modified Viñas, or Nike; I don’t perform endoscopic brow lifts or foxy eyes.

- Anesthesia: total intravenous anesthesia, in a hospital; without a facelift, you can go home the same day.

- Recovery: in the literature, people typically return to normal activities in 10 to 14 days; your timeline follows the combined surgery.

- Scar: along the edge of the brow; not invisible.

- Risks: numbness, asymmetry, hair loss, and the need for revision; results are not guaranteed.

Your eyes

## What do you notice when you look at your eyes?

You may not think of it as a low brow. You might just notice that your eyes look heavier, that your eye makeup no longer shows, or that your face looks more tired than you feel. I want to start by hearing how you see it, before putting a name to any procedure.

During the exam, I look at where your brows sit with your forehead relaxed. Some people raise their forehead almost without realizing it to offset the heaviness. I also assess the height of the eyelid margin and any excess skin. These details are closely related, but treating one doesn’t take care of the other.

### Skin folds over the eyelid

Blepharoplasty may be the focus when there is excess skin and the brow is well positioned.

### The tail of the brow has dropped

Adjusting the brow may be part of the plan if the position of its outer part adds to what bothers you.

### The eyelid margin sits low

Eyelid ptosis needs to be investigated; removing skin or lifting the brow won’t automatically fix it.

Planning

## The expression I want to preserve is yours

I’ll ask you to describe what you’d like to get back: a less heavy outer brow, more visible eyelids, or better balance between the two sides. Old photos can help with this conversation. They help me understand how your face has changed, not set a rule that every youthful proportion has to be recreated.

I plan around the brow, the eye socket, the skin, and any existing asymmetry. A brow height that looks good on someone else can give you an odd expression. I make a point of discussing the change both at rest and in motion, and of respecting what you don’t want changed.

If treating only the eyelid is the better answer, I’ll explain why. If a brow lift makes sense, I’ll show you which part of the brow I plan to adjust and where the incision will be. That way, you decide knowing what each step contributes.

Techniques

## Three incision patterns for different needs

My approach focuses on the outer part of the brow and is combined with upper blepharoplasty. The choice among the patterns below depends on your exam; this isn’t a ranking in which one is more complete or better for everyone.

In my practice, a brow lift is not done as a standalone surgery. I also don’t perform the endoscopic technique or the procedure known as foxy eyes.

### Mini-Castañares

Lets me focus the adjustment on the transition between the middle and outer parts of the brow.

### Modified Viñas

Targets mainly the tail, when the droop is concentrated at the outer end of the brow.

### Nike

Combines the two patterns when the brow needs to be repositioned over a wider outer area.

The scar lies close to the edge of the brow. How visible it is depends on, among other things, how dense your brow hair is, your skin, and how you heal. I don’t present it as invisible. Before surgery, this trade-off between repositioning and scarring needs to be clear.

### Want to find out what’s weighing down your eyes?

At the consultation, I’ll sort out what comes from the eyelids and what comes from the brows, and show you which changes make sense for you.

Surgery

## Anesthesia, where surgery takes place, and your quote

Because the brow lift goes along with upper blepharoplasty, the operating time is planned for the combined procedure. Surgery takes place in a hospital in Londrina, under total intravenous anesthesia (TIVA), the same anesthesia I use for almost all my surgeries. [The ASPS describes the steps of the surgery](https://www.plasticsurgery.org/cosmetic-procedures/brow-lift/procedure). When the combination is done without a facelift, you can go home the same day if you wish: since I schedule these cases for the afternoon, discharge is at around 10 p.m.

Your quote reflects the combination: the blepharoplasty, the part of the brow treated, and the hospital facilities and anesthesia. See how it works on the [pricing](https://www.walterzamarianjr.com/us/pricing/) page. On WhatsApp, your first contact is with Bruna, my AI assistant, who then hands you over to Isanna. If you live far from Londrina, you can start with an online consultation. I only see private-pay patients.

With a facelift

## How this fits with a facelift

A [deep plane facelift](https://www.walterzamarianjr.com/us/face/facelift/) and treatment of the eye area have complementary goals. Surgery aimed at the midface and jawline shouldn’t be expected to correct every sign of aging around the eyes.

When I see someone who wants broader rejuvenation, I keep the decisions separate: what calls for repositioning the face, what belongs to the eyelids, and what truly depends on the brow. Combining procedures may make sense, but that doesn’t mean a fixed list of operations is required.

You can read the [blepharoplasty](https://www.walterzamarianjr.com/us/face/blepharoplasty/) guide to better understand the difference. At the consultation, I show you how this distinction applies to your face, without expecting you to arrive knowing which surgery to choose.

Evidence

## My choice of technique and what the research can tell us

My preference for the open approach is a choice I make in my practice, not proof that it is superior in every setting. Other surgeons work with endoscopic techniques and other ways of repositioning the brow.

A review published in 2026 included 15 studies and found results favoring endoscopic or less invasive methods in some comparisons. The authors themselves point to major differences between the studies and the need for better prospective research. These data make it impossible to turn a personal preference into a universal rule. [Read the 2026 comparative review](https://pubmed.ncbi.nlm.nih.gov/41710188/).

The review by Cho, Carboy, and Rohrich, for its part, describes different complication profiles depending on the technique. In practical terms, this underscores the importance of discussing scarring, sensation, and the possibility of revision with the surgeon who will actually perform the operation. [Review of complications](https://pubmed.ncbi.nlm.nih.gov/30534495/).

The 2009 paper on a modification of the Castañares technique described 350 cases, many of them combined with blepharoplasty or a facelift. It is a technical reference for understanding the direct approach, not a comparison that proves superiority. The variations I use are chosen based on each person’s exam. [Clinical series of the modified technique](https://pubmed.ncbi.nlm.nih.gov/19881022/).

Alternatives

## Other ways to treat the brow

Knowing the alternatives helps you understand why one of them was chosen for you. Some I perform; others I don’t, and I’d rather say so clearly.

### Botulinum toxin

I don’t perform botulinum toxin injections. In selected cases, it can slightly raise the tail of the brow, with a temporary effect. It doesn’t remove skin or reposition tissue.

### Endoscopic brow lift

Done through small incisions in the scalp, with the help of a small camera. I don’t perform this technique.

### Hairline or temple incisions

Other surgeons use them to lift the forehead or the outer brow. The scar sits in the scalp or along the hairline, with its own risks, such as hair loss near the incision.

### Blepharoplasty alone

When the brow is well positioned, treating only the eyelid is usually the way to go.

Recovery

## Planning the first few days

Your recovery follows that of the blepharoplasty and of any other procedures done at the same time. Plan for a stretch with fewer in-person commitments, since swelling, bruising, and a scar that is noticeable at first may bother you more socially than pain does. How things look in the first few weeks doesn’t determine the final scar.

For reference, the ASPS says you should be ready to return to work and normal activities 10 to 14 days after a brow lift, with the scar and swelling continuing to improve over the following weeks. The same guidance calls for keeping your head elevated and avoiding strenuous physical activity for as long as your surgeon recommends. [See the ASPS guidance on recovery](https://www.plasticsurgery.org/cosmetic-procedures/brow-lift/recovery). In your case, the timeline is set by the combined surgery.

Before you go home, you’ll receive a plan for hygiene, medications, and follow-up visits. When you can use makeup and other products on the area depends on how you are healing. You can resume light exercise after 1 month and any physical activity, including contact sports, after 2 months. Don’t try massage or other treatments on the incision on your own.

At follow-up visits, I check the scar, the position of the brows, and how well your eyes close. Follow-up continues for one year. The [Journey](https://www.walterzamarianjr.com/us/patient-journey/) page shows how the team organizes follow-up care; when surgeries are combined, the schedule for the combination applies, not the shortest timeline you might find for a single procedure.

Risks

## What needs to be part of your decision

Risks include bleeding, infection, a noticeable or widened scar, a difference in height between the two sides, changes in sensation, hair loss near the incision, and the need for revision. The correction may fall short, or the brow may end up higher than you wanted. The evaluation should also take into account the risks of the blepharoplasty done at the same time.

Numbers from the literature help put these risks in perspective. In a review of 76 studies, the direct brow lift, done along the brow, had the highest rate of numbness among the techniques, 5.5% ([Cho, Carboy, and Rohrich, 2018](https://pubmed.ncbi.nlm.nih.gov/30534495/)). Another review, of 82 case series, found unsatisfactory scarring and changes in sensation to be the most common complications of surgical brow lifts ([Byun and colleagues, 2013](https://pubmed.ncbi.nlm.nih.gov/23388642/)). In the series of 350 cases using the modified Castañares technique, the most frequent complications were epidermal cysts, in 3.4%, and wound separation, in 2% ([Viana and Viana, 2009](https://pubmed.ncbi.nlm.nih.gov/19881022/)). The ASPS also lists, among possible risks, injury to a branch of the facial nerve with forehead weakness. [See the full ASPS list of risks](https://www.plasticsurgery.org/cosmetic-procedures/brow-lift/safety).

The repositioning is long-lasting, but it doesn’t stop aging: over the years, the brow may drop again. Results are not guaranteed.

Let me know if you have a history of dry eye, eye disease, previous surgery, or unfavorable scarring. If you have severe eye pain, a change in vision, or rapidly increasing swelling after surgery, seek urgent care without waiting for your routine follow-up visit. Shortness of breath or chest pain needs emergency care right away, without waiting for a reply to a message: these are rare signs of a blood clot that has traveled to the lungs (pulmonary embolism), which the ASPS includes in its list of risks.

Knowing these points helps you decide with peace of mind. My goal is for you to leave the consultation understanding the intended change and the care you’ll need to get through recovery.

Questions

## Your questions deserve a careful answer.

Will I look surprised?

Lifting the brow too much can change your expression. That’s why the pattern and extent are discussed beforehand, taking into account where your brows start and how they move. The goal is to relieve the heaviness, not to arch the brow as high as possible.

Can men have a brow lift too?

Yes, when it’s indicated, but the brow shape we aim for is different. In a study of photographs of 300 young, White, male professional models, the male eyebrow was straight and low, sitting at the orbital rim (Pietruski and Majak, 2023). That’s why, in men, the goal is to relieve the heaviness without arching the brow.

Can I have just a brow lift with you?

In my practice, this adjustment is done together with upper blepharoplasty, when it is indicated. The consultation will clarify whether this plan fits your needs.

Does botulinum toxin do the same thing?

I don’t perform botulinum toxin injections. In selected cases, it can temporarily change the balance between the muscles. It isn’t the same as surgically removing and repositioning tissue.

Does a brow lift treat forehead wrinkles?

That isn’t the goal of the approach I use. The ASPS describes an improvement in forehead lines and in the lines between the brows with a forehead lift, which addresses the whole area. The techniques I perform target the outer part of the brow and are meant to reposition it, not to treat those lines. For expression lines, botulinum toxin injections are one of the options; I don’t offer them.

Will both brows look exactly the same?

Any existing asymmetry needs to be recognized beforehand. I aim to improve balance, but perfect symmetry isn’t a realistic expectation.

How do I get a quote?

Once the plan for the blepharoplasty and the brow adjustment is set, the team gives you the corresponding quote. You can go over each item and plan your recovery before deciding.

How long does recovery take?

In the literature, people usually return to work within 10 to 14 days. Because I perform the brow lift together with upper blepharoplasty, the timeline of the combined surgery applies, and it is set at the consultation.

Can the brow drop again?

Yes. The result is long-lasting, but aging continues, and the brow can drop again over the years.

Tell me how you see your eyes. I’ll look at how your brows and eyelids relate to each other before suggesting any change.
