# Dark Circles and Under-Eye Bags: When Can Surgery Help?

Color, shadow or volume? What eyelid surgery can treat for dark circles and under-eye bags, and what may remain afterward.

## At a glance: when can surgery help with dark circles and under-eye bags?

Surgery treats bulging fat, excess skin and, with a fat graft, hollows under the eyes. It does not remove every dark shade: pigmentation, thin skin, visible vessels and shadows often combine. That is why I separate these features at the examination before proposing lower blepharoplasty, and explain what may remain afterward.

- Bags: bulging fat that can be reduced by removing the excess.

- Deep hollows: a depression that can be treated with a fat graft such as nanofat.

- Color: pigment and vessels don’t go away when bags or skin are removed.

- Technique: transconjunctival approach without extra skin; pinch technique when there is extra skin.

- Festoons: conventional blepharoplasty does not automatically correct them.

- Limits: I don’t give botulinum toxin or filler injections.

My [lower blepharoplasty](https://www.walterzamarianjr.com/us/face/lower-blepharoplasty/) page covers the technique, recovery and risks of lower eyelid surgery. This article starts one step earlier: is what bothers you under your eyes something surgery is meant to treat?

You may sleep well and still hear that you look tired. I begin by asking what bothers you most: the darkness, a bulge, a hollow or the combination. “Dark circles” describes what you see, but it is not a diagnosis on its own.

## Four observations to bring to the consultation

### Is color more bothersome than shape?

Point to the area that catches your eye, without trying to classify it yourself. The review by Roh and Chung describes pigmentation, skin transparency and shadows among the possible contributors [2]. One photo can’t tell which one dominates.

### Do you see a bulge or a hollow?

A bulge and a depression are different concerns, even when they sit side by side. Show me what you call a bag or a dark circle, so I don’t explain one structure while you are worried about another.

### Does it change during the day?

Tell me when you notice it most and whether it changed recently. That history helps, but it is not a home test that separates fat from swelling.

### Has the area been treated before?

Tell me about previous surgery, fillers and other procedures in the area, even old ones. If you have dates, product names or records, bring them: they change the plan.

## Why dark circles and bags are not the same thing

In everyday speech the words overlap: one person calls anything that looks tired dark circles; another calls any contour under the eyes a bag. The American Academy of Ophthalmology notes that loose skin, fat, pigmentation and natural shadowing can all produce dark circles or “bags under your eyes” [4].

The literature on darkness around the eyes recognizes pigment-related, vascular, structural and mixed causes [1]. That explains why a treatment that answers one person’s concern may not answer another’s. The question is not only whether a treatment exists, but which feature it is meant to address.

## What surgery can treat

### Under-eye bags

I remove the excess fat from the bags without hollowing the area out. When there is no extra skin, the approach can be from inside the eyelid, with [transconjunctival blepharoplasty](https://www.walterzamarianjr.com/us/face/transconjunctival-blepharoplasty/), and no skin incision.

### Excess skin

When there is extra skin, I use the pinch technique with an incision slightly below the lashes, not right at the lash line, to help prevent ectropion. The amount removed has to leave the eyelid able to protect the eye.

### Deep hollows

For hollows, I use nanofat. I repositioned fat in the past, but the results I got were not as good, so I stopped; that is my own experience, not a judgment of surgeons who reposition it.

I operate in a hospital, under total intravenous anesthesia (TIVA). Eyelid surgery takes about 1 hour on average; done without a facelift, it allows same-day discharge if you wish, at around 10 p.m.

## What surgery does not fix

A systematic review published in 2021 included 39 studies of treatments for darkness around the eyes. The authors matched the approaches to the main causes and noted the lack of high-quality evidence [1]. The treatments in a review are not a menu that works for every dark circle.

Improving contour is not the same as removing pigment. Roh and Chung describe contour and laxity as contributors to under-eye shadows, while pigmentation and skin transparency are separate causes [2]. I can plan a change in contour and still tell you that some of the darkness may remain.

I don’t give botulinum toxin or filler injections, and I don’t perform laser treatments or peels around the eyes: what I use is nanofat grafting. If what bothers you calls for a dermatologist’s assessment, I would rather say so than stretch the indication for surgery.

A useful question for the consultation is: “Even if the bag improves, could this darker area still be there?” It lets us discuss what the examination showed and what cannot be predicted.

## Where nanofat fits

For selected hollows under the eyes, I prefer nanofat because of how gently it settles in this thin skin. That is a choice from my own practice, not proof that it lightens every kind of pigmentation or fixes every tired look. In the original study by Tonnard and colleagues, the clinical focus was skin quality, not structural volume replacement [5].

The article on [nanofat and skin quality](https://www.walterzamarianjr.com/us/blog/nanofat-and-skin-quality-what-the-research-can-tell-you/) explains what the studies measured and where my experience comes in.

## What about swelling lower down, toward the cheek?

Not every bulge in this area is a lower-eyelid bag. A 2026 review by Quah and Ezra discusses malar edema, malar mounds and festoons as conditions with overlapping anatomy and causes, and stresses limited evidence and the need for individualized care [3].

So I don’t promise that conventional blepharoplasty will correct a festoon. The location and nature of the finding need examination, and I don’t claim to offer every approach described in the literature.

## Before-and-after photos and online consultations

When looking at results, ask which procedures were done and how long afterward the photo was taken. A combined result can’t show what came from bag removal or from the graft alone, and lighting and angle change the visible shadow.

If you live abroad, a first online consultation helps organize records and questions, but the plan is confirmed in person before surgery. The [patient journey](https://www.walterzamarianjr.com/us/patient-journey/) explains that sequence. A selfie is not enough to make a diagnosis.

## Five questions for a clearer decision

- What dominates in my case: color, contour, skin or a combination?

- Which of these is the proposed procedure meant to treat?

- What might still bother me after surgery?

- Should another specialist see me before I decide?

- Which risks and limits apply to my case?

## Frequently asked questions

Does a purple or brown color tell me which treatment I need?

Not on its own. Lighting, photos and mixed causes affect what you see. I use the examination and your history to understand the finding.

If I remove the bags, will the whole area get lighter?

I can’t promise that. Improving the bags is separate from the other causes of darkness, and I explain beforehand what may remain.

Do you inject filler for dark circles?

No. I don’t give filler or botulinum toxin injections. For deep hollows, when surgery is indicated, I use a fat graft (nanofat).

Do I need a facelift to treat my eyelids?

No. I assess your concern and your face as a whole; an eyelid concern does not, on its own, call for a larger operation.

Can I book a consultation before deciding on surgery?

Yes. The consultation is for understanding your options and limits, and it doesn’t require a prior decision to have surgery.

## Let’s work out what is bothering you

If you are not sure whether you are seeing a bag, a shadow or a skin change, [write to us on WhatsApp](https://www.walterzamarianjr.com/us/contact/): Bruna, my AI assistant, handles the first contact and then puts you in touch with Isanna to arrange a consultation with me. We start with that distinction and a plan based on your examination, without promising that one technique will solve everything.
