Dr. Walter Zamarian Jr. is a member of the ASPS (American Society of Plastic Surgeons) and the AExPI (Association of Alumni of Professor Ivo Pitanguy). He has worked at the Burn Units of the University Hospital of Londrina and the Andaraí Hospital in Rio de Janeiro. His clinic is one of the leading destinations in Londrina, Paraná.

He graduated in Medicine from the State University of Londrina, where he also completed a two-year residency in General Surgery. Dr. Zamarian Jr. gained an international perspective on plastic surgery during his three years of training under renowned surgeons from various countries in Rio de Janeiro, during his postgraduate studies at the Ivo Pitanguy Clinic and at the 38th Ward of the Santa Casa de Misericórdia in Rio de Janeiro. However, we can go further.

Another more comprehensive definition was mentioned by a resident physician as "surgery of the skin and its contents." Since plastic surgery is not directed at a specific organ, it knows no boundaries, and the chosen technique varies according to each case. Thus, surgery is endowed with freedom, but at the same time, vulnerability. It is precisely this vulnerability that drives the surgeon to constantly pursue excellence in their areas of expertise, constantly seeking to improve their techniques and develop innovative approaches.

Plastic surgery is more based on principles than on specific procedure details. This allows the surgeon to address uncommon problems and perform surgery from the hairline to the tip of the toe, applying well-known procedures to various areas of the body, allowing for innovation.

No other specialty receives the same media attention as plastic surgery. At the same time, none is so poorly understood. Although the public perceives plastic surgery as primarily cosmetic, it has its roots in reconstructive surgery. The Evolution of Aesthetic Surgery

After years in second place, it happened as many had predicted: Brazil took the lead in the number of plastic surgeries worldwide. As the demand grows, so does the number of professionals. Today, there is approximately one plastic surgeon for every 44,000 inhabitants in the country. The data shows that there are more doctors with this specialization proportionally even compared to the United States, the country with the second-highest number of procedures. In the U.S., there is one specialist in the field for every 50,000 inhabitants.

The interest and demand for plastic procedures continue to increase year after year. To give you an idea of the phenomenon: in 2011, there were 905,000 plastic surgeries performed in Brazil. In 2013, the number rose to 1.49 million, accounting for nearly 13% of the global total. That's right. Through the hands of surgeons, many lives have been transformed, restoring self-esteem, femininity, self-confidence, finding peace with one's own body, and even reconstructing the marks of a not always happy history. And this has been happening for millennia, even before the time of Christ.

The earliest references found for reconstructive treatments date back to the period between 3,000 and 2,500 BC Egyptian papyri described the treatment of nasal fractures. The techniques for nasal reconstruction date back to 800 BCE. It was in India that they had the first experiences with Sushruta Samhita, the father of plastic surgery, who wrote the medical treatise bearing his name. In the document, he explained a traditional procedure for performing reconstructive surgery of the nose. His techniques for rhinoplasty crossed time and cultures. The instruments described in the Sushruta Samhita were modified and perfected in the Western world.

The Indian surgeon developed surgical techniques to reconstruct noses that were amputated as religious, criminal, or military punishments, which was common during that period. These techniques were also used to restore the noses of soldiers injured in battle.

In fact, wars motivated the development of new reconstruction techniques millennia later. We can say that the history of plastic surgery also has a strong connection with the history of post-war resilience in the early 20th century. Soldiers mutilated in World War I found a path to recovery and redemption through the hands of New Zealand doctor Sir Harold Gillies. It was in 1916 when he performed the first surgery on soldier Willie Vicarage, who had been shot in the face. As a pioneer of these transformations, Gillies is considered the father of modern plastic surgery. He restored harmony to many faces destroyed in combat, paving the way to heal the soul as well, by alleviating that physical pain.

Beauty is something humanity has sought to emphasize since ancient times. Even before the time of Christ, we find manifestations in the pursuit of beauty. Body paintings, necklaces, historical records of reconstructions: Indian physicians performed skin transplants and nasal reconstructions as early as the 8th century BC, a period when nose amputation was a punishment for certain crimes. These practices were further refined with the development of medicine and, most notably, the discovery of general anesthesia in 1846 by William T.G. Morton.

The fertile ground was set for plastic surgery to be born and captivate doctors and patients worldwide. The art of shaping and reshaping any area of the body is now performed on every continent, each respecting the culture and habits of each country.

Techniques and Procedures of the Major Plastic Surgeries Worldwide

Brazil holds the top position in the absolute number of plastic surgeries performed. Our country is the global leader in buttock implant surgeries (known as gluteoplasty), vaginal rejuvenation (intimate surgery), and surgery to correct protruding ears (otoplasty). In 2011, there were 55,746 intimate surgeries worldwide – with 9,043 of them performed in Brazil (over 16%). Speaking of buttock augmentation surgeries, there were 75,591 performed worldwide, with 21,452 in Brazil, representing 28% of the total.

However, the champion in national preference is still liposuction, which reshapes the body, followed by breast augmentation surgery with silicone implants. In 2011, there were 211,108 liposuctions and 148,962 breast augmentation surgeries. Another procedure that is gaining prominence here is blepharoplasty (eyelid surgery). In 2011, the number of surgeries using this technique reached 90,281.

Considering all the procedures combined worldwide, liposuction also takes the lead in preference, reaching up to 20% of all plastic surgical procedures and reaching a total of 1,268,287 procedures in 2011. The technique is relatively recent and can be associated with abdominoplasty, which involves removing excess skin from the abdomen. In Brazil, the first liposuction occurred in 1980, performed by French surgeon Yves Gerard Illouz, a pioneer of liposuction. During the procedure, 4 liters of localized fat were removed from the thighs. The first liposuction performed in the world dates back to 1978, performed, of course, by its creator, Dr. Yves Illouz, in France. Since then, millions of cosmetic surgical procedures involving liposuction have been performed, fulfilling the desires of many patients with excess localized fat.

Looking at individual countries, there are cultural curiosities. In China, for example, liposuction is also the most performed surgery, but the country leads in the number of rhinoplasties (nose surgeries). In 2011, 51,680 procedures modified this body segment in the Chinese population. Japan follows in second place with 46,599 nose surgeries. In the United States, the most sought-after surgery is breast augmentation, with 284,351 procedures of this nature performed in 2011. In France, breast augmentation also takes the first place with 41,484 procedures.

The technique of breast augmentation using silicone implants, with or without mastopexy, has now reached 50 years. The first woman to undergo this procedure was American Timmie Jean Lindsey in 1962.

All numerical data were extracted from a research conducted by the International Society of Aesthetic Plastic Surgery (ISAPS), involving the 25 countries where the most plastic surgeries are performed worldwide. The base year is 2011.

In Brazil, it is not mandatory to be a plastic surgeon to perform surgery; being a medical doctor is sufficient.
